Always, always use jack stands when under vehicles or any type of machinery that been jacked up, hydraulics do fail or slide out.

This as much a sad incident as it should have been avoidable.
Firstly, this should serve as a reminder to absolutely everyone who uses a jack a lever or any other means to hoist a vehicle ... always use a sturdy block or a brace or both to lower a vehicle upon before getting underneath to do an inspection or any work. Even better to have another person in close proximity who can intervene should an accident occur. Time is essential in these dire circumstances. No doubt, if the deceased person was an employee, the employer will undergo a sever audit of their safety training and procedures. by WorkSafeBC.

RIP Amandio you were one of the best orchardist I have ever met. I remember working alongside you, picking apples, and there you were picking apples next to me and Mark in your old age. Your orchard was always pruned right, with easy access to fruit, and your apples were always top notch. You were such a hard worker and you left a great legacy to your children and grandchildren.
