Only thing the employee pays for is life and disability to keep the benefit non-taxable, which is common for most plans.
The rest is 100% company paid.

Yup.
They wanted something beyond basic wages and
Having to pay for life and disability insurance all by themselves.
Very unusual for the employer to pay all of those nowadays.
